ABSTRACT

 

Title: Extraction of one lower incisor-always an easy lingual case?

 

Aims: Crowdings and malformations of the teeth in the lower jaw are frequent especially in the anterior region. Diagnosis leads to treatment plans, which may vary, from changing the intercanine distance, slicing or extraction therapy. When it comes to extraction therapy sometimes one lower incisor needs to be extracted. Lingual appliances design may be different to labial bracket design when it comes to the bracket slots and wire insertions. In order to work with flat lingual appliances, a vertical insertion of the wire in the anterior is frequently used. The purpose of the presentation is to demonstrate clinical and biomechanical aspects and challenges on lingual cases with extraction therapy of one lower incisor.

 

Methods: Based on different orthodontic cases, different indications of extracting one lower incisor will be discussed. Mechanics for space closure will be demonstrated. The purpose of esthetic outcomes based on physilogical and periodontal new positioning of the incisors will be described. In some cases interdisciplinary treatments are required.

 

Results: Treating lingual cases with extraction of one lower incisor can lead to state of the art outcomes, if the practitioner respects certain biomechanical rools using the right mechanics to achieve the correct final position of the incisors.
